Requirement

Individual/ Incorporation Company

1. Fill out the application form
2. Photocopy of ID Card of Applicant
3. Photocopy of the proof of the land / field ownership where the place of business established
4. Statement letter of no objection from the neighborhood that are known to the local village chief and district head
5. Picture of a situation of place business
6. Statement letter of the ability to manage and control the environmental impact
7. Statement Letter of the ability to obey the determination of laws and regulations

Corporation Company

1. Complete the Application Form
2. Photocopy of ID Card of Applicant
3. Photocopy of ownership the land / field proof  where the business place is established
4. Statement letter of not Objection from the neighborhood that are known to the Local village chief and District Head
5. Photocopy of Company Establishment Deed
6. Photocopy of Tax Registration Number
7. Picture of a situation of the business place
8. Statement Letter of the ability of managing and controlling the   environmental impact
9. Statement Letter of the ability of obeying the determination of laws and regulations.

Processing Time and Fee

1. Processing Time is 9 days

2. Retribution Fee is counted by the formulation :
