Requirements

1. Fill the request form.
2. Photocopy of Director / Leader / Owner's ID.
3. Business License / Technical License of Changing Certificate.
4. Photocopy of Establishment Certificate, the Change for the Legal incorporated Company that is ratified by the authorized Institution.
5. Photocopy of Judicial Certificate (establishment, change, judiciary) special for PT (Limited Corporation)
6. Photocopy of Changing Certificate for all Legal Entities(PT, CV)
7. Photocopy of Establishment Certificate legally by Perindagkop( Industry and Trading Department)(Special for Cooperation)
8. Photocopy of New RAT (Special for Cooperation)
9. Photocopy of Balance Sheet (Neraca) for all Legal Entities (PT,CV,Cooperation, FA)
10. Photocopy of Number of Tax Payer (for all companies which have a legal entity).
11. Photocopy Disturbance License (for all companies which have an environmental impact).
12. Legacy Stamp Rp 6.000,-
13. Photocopy of The Technical License.
14. Photocopy of Business Place Status (Certificate, Rental, Contract or others)
