Requirements

• Fill the request form.
• Photocopy of the Businessman ID
• Photography of the Businessman sized 3 cm X 4 cm = 2 pcs (for every product/trade mark).
• Location and Room plan drawing.
• Production Flow  (completed with used materials composition)
• Sample of Label/packging/product mark
• Laboratory Result of Drink Food sample which is qualified health.
• Have a Feasible Hygiene Sanitation Certificate.
• Businessman/employee has followed the Food Safety Counseling (Certificate of Food Safety Counseling).
• Photocopy of past SPPIRT for Re-registration.
