Requirements
- Photocopy of the Establishing Certificate of Foundation legally incorporated from Notary ( if it is legal entity ).
- For the personal request is needed a working experience was ratified by the institution Leader where he worked.
- Photocopy of the Disturbance License.
- Location Permits / Recommendation from the Healthy Institution Leader.
- List of Healthy Worker and Service Organization Structure which is described in the duties and functions.
- Photocopy of Technical Executives, Paramedical and Doctor Practical License.
- Photocopy of Professional worker Certificate.
- Location and Building Plan Drawing.
- Drugs and or Cosmetics List which is available.
- Statement Letter of obeying to the institution rules (legacy stamp).
- Disturbance License (HO).
