Requirements
- Photocopy of Legal Foundation Establishment Certificate from Notary( especially which has a legal entity ).
- For the personal request is needed the Work Experience which is ratified by the Authorized Institution Leader where he worked.
- Photocopy of Disturbance License.
- Location Permits / Recommendation from the Healthy Service Leader.
- Organization Structure of the Service with the duty and function description.
- Photocopy of paramedical and doctor practical license.
- Location and Building Plan Drawing.
- Available Drugs List in Medical Clinic.
- Statemanet Letter of obeying to the valid rules and institution (legacy stamp).
- Disturbance License (HO).
