Requirements
- Photocopy of the Company establishing certificate from Notary ( if it's a legal entity).
- For the Personal Request is needed a Work Experience was ratified by the Institution Leader where he worked.
- Photocopy of the Refraction Optician (RO) as an Optical Responsibility.
- Photocopy of Disturbance License, Location Permits / Recommendation from the Leader of Healthy Service.
- RO Statement Letter that the person involved can be an Optical Responsibility.
- List of Healthy Workers and Service Organization structure is described in the duties and functions.
- Photocopy of the Working licence of Refractionist Optisience (RO) from the Healty Service .
- Location and Building Plan Drawing.
- List of the Used Equipments .
- Statement Letter of obeying the Law and Rules (Legacy Stamp).
- Disturbance License (HO).
