Requirements
- Fill the request form.
- Photocopy of the Established Certificate of the Foundation which has a legal entity from Notory ( Special for a legal entity Company).
- For the personal request, it's needed a job experience history is ratified by the Leader of Institution where he worked.
- Photocopy of the Disturbance License.
- Location License/Recommendation from the Leader of Healthy Service.
- Table of Healthy Workers and the Organization Structure of Service which is described in the duty and function definition.
- Photocopy of the Doctor Practical License and Paramedical .
- Location plan drawing and building plan drawing.
- Available Medicine Draft in Childbirth Clinic.
- Statement Letter that will be obeyed to the valid institutions rules (Legacy Stamp) .
- Disturbance License (HO).
