Requirements

1. New Route License  / New Route Open
   • Fill the Request form.
   • Copy of a Letter Mark Number of Vehicle ( public ).
   • Copy of the Test Book.
   • Copy of ID.
   • Copy of Business License .
   • Recommendation / Considering Letter from Transportation Service (Dinas Perhubungan).
2. Re-registration The Validity Period of Route Licensing Decision
   • Fill the request form.
   • Decision Letter of Route License.
   • Copy of the Test Book
   • Copy of ID.
   • Copy of Business License .
   • Copy of vehicle number.
   • Recommendation / Considering Letter from Tranportation Service (Dinas Perhubungan).
3. Re-registration the Validity Period of Control Card.
   • Fill the request form.
   • Copy of Route License.
   • Copy of a Mark Letter of Vehicle Number .
   • Copy of Test Book.
   • Copy of ID.
4. Regeneration of Vehicle
   • Fill the request form.
   • Copy of Old Vehicle Number.
   • Copy of New Vehicle Number.
   • Copy of Test Book.
   • Copy of ID.
   • Copy of Business License.
   • SK/ Decision Letter of the Route License with the attachments.
   • Recommendation/Considering Letter from Transportation Service(Dinas Perhubungan).
5. Transfer License of Company Ownership
   • Fill the request form.
   • Copy of old Vehicle Number.
   • Copy of new vehicle number.
   • Copy of Test Book of New Vehicle.
   • Copy of ID of Old Owner and New Owner.
   • Copy of Business License(New Owner).
   • Recommendation from Transportation Service .
   • Certificate of Route License with the attachments.
   • Statement Letter from Old Vehicle Owner to New Vehicle Owner.
6. Replacement of Licensing Document that's lose
   • Certificate of Lost from Police Department.
   • Copy of vehicle Number, Test Book, Route License and the attachments.
