Regional Rules number 10 Year 2006 about industrial Business License

Requirements :

1. Fill  the request form.
2. Photocopy of NPWP/NPWPD (Tax Number)
3. Photocopy of Building and Land tax
4. Photocopy of HO/Neighborhood Statement.
5. Photocopy of Feasible Hygiene Sanitation (Food, Drink).
6. Photocopy of :
   • Certificate of Establishment and Change (CV,PT)
   • Establishment Certificate ratified by the Perindagkop ( Cooperation) for the production.
7. Document of UKL-UPL/SPPL.
8. Judicial Establishment + Judical Change Certificate (PT).
9. For Business License of water Refill, there must be a water supply from PDAM (Water Company).
